Collaborative Care is an evidence-based model to identify and treat patients with depression and anxiety in healthcare settings. Adopted by hundreds of health centers across the U.S., Collaborative Care is a Medicare and Medicaid benefit in 24 states. The model assists health centers with managing the population of patients with depression and anxiety, addressing UDS measurements, and managing behavioral health workforce challenges.
