Webinar: Transforming Early Childhood Health Care in Community Health Centers

In collaboration with parents, healthcare providers, and child health experts, Nurture Connection has piloted an Early Childhood Health Transformation Change Package for HRSA‘s Early Childhood Development (ECD) Community Health Center’s grant recipients. This initiative, known as Nurturing Connection for Community Health Centers (NC4CHC), is designed to align care delivery with Bright Futures guidelines while emphasizing ERH and is informed by the Guide for Transforming Care of Young Children in Community Health Centers.
The NC4CHC framework focuses on three key areas: 1) screening: Increasing the rate and effectiveness of early childhood developmental screenings, 2) referral and Follow-up: Strengthening care coordination strategies to enhance outcomes, and 3) family partnership — deepening family engagement and collaboration in care delivery.
