Patient Education
ACU Diabetes Patient Education Series
ᅠ
Through a partnership with Home Diagnostics, a producer of glucometers, the ACU has created new patient education materials focused on diabetes for a low literate and multicultural population. All were pilot tested in health clinics with patients served by ACU members. If you are interested in pilot testing low literacy materials developed by the ACU in your site, please contact Lois Wessel, CFNP.
Each flyer is available in tear pads of 25 sheets. To order, please use this order form (PDF).

Other Sources for Patient Education Resources
The Agency for Healthcare Research and Quality (AHRQ) offers free clinician and consumer guides that compare the benefits and risks of medications and/or other therapies for a wide range of health problems such as type 2 diabetes, gestational diabetes, osteoarthritis, osteoporosis, rheumatoid arthritis, depression, and rental artery stenosis. The guides are based on the results of AHRQs comparative effectiveness research program. AHRQ is part of the U.S. Department of Health and Human Services. The consumer guides are available in English and Spanish.
Ambulatory care practices and hospitals may request up to 500 free copies of the clinician and consumer guides, and individual clinicians up to 200 copies, stock levels permitting. Audio versions of many guides also are available.
To order, call the AHRQ Publications Clearinghouse at 1-800-358-9295 or send an e-mail to ahrqpubs@ahrq.hhs.gov.
